Kearney Combat Sports LLC  Participation Waiver

I _________________, understand that there are many risks in the participation of every kind of martial arts activities. Martial arts such as boxing, kickboxing, jiu-jitsu, wrestling, and MMA are all included in my understanding and therefore I willingly give up my rights to hold Kearney Combat Sports LLC and any other entities associated with Kearney Combat Sports LLC responsible or liable for any injuries or damage I may suffer while participating in Kearney Combat Sports team practices, training sessions, or classes. I will also not hold Kearney Combat Sports LLC responsible for anything that may happen to me as I am spectating, solely visiting the premises, or leaving the premises after a class or training session at Kearney Combat Sport LLC.

     In fully understanding the aforementioned risks pertaining to the participation of all Martial arts activities and being permitted to take part in any classes, training sessions, and/or team practices held by Kearney Combat Sports LLC, I voluntarily waiver my rights to ever hold Kearney Combat Sports LLC liable for anything that I might incur. Neither will I hold any associated entities with Kearney Combat Sports LLC accountable for anything that I might incur during my time at the premises or as a participating member in Kearney Combat Sports LLC. I also understand that the classes/training in sessions/practices that are offered at Kearney Combat Sports LLC include a variety of different high risk activities such as; throwing punches, kicking, throwing elbows and knees, submission holds to the legs and arms, choke holds, falling, stretching...etc. I understand that the participation in these kinds of activities is potentially dangerous. In fully knowing the potential danger that has been described I am of my own will choosing to take part in the classes/training sessions/practices at Kearney Combat Sports LLC, and agree to assume the risks associated with my decision to participate in any shape or form therein.

     I hereby am of the understanding that I am assuming full responsibility for any and all risks of death, injury, emotional suffering, property damage, and economic loss that I might incur as a result of my participation in any way, shape, or form with Kearney Combat Sports LLC. I acknowledge that I am in agreement with this waiver and aforementioned conditions and I, my spouse, heirs, personal representatives, agents, my children and their agents are bound to this agreement. I hereby also agree to release, indemnify and hold completely harmless Kearney Combat Sports LLC and any other entities associated with Kearney Combat Sports LLC including owners,  members, employees, heirs, independent contractors harmless from all liability. Included are costs, attorney fees, stemming from any kind of participation with/in/at the premises of Kearney Combat Sports LLC. I understand that this includes waiting or walking through the premises before or after class hours at Kearney Combat Sports LLC. I acknowledge that if I am to file a lawsuit against Kearney Combat Sports LLC or any associated entities with its owners,  members, employees, independent contractors, as a result from incurring personal injuries, death, emotional pain and suffering, economic loss, property damage suffered on my part by my participation in the martial arts training/classes/practices held by Kearney Combat Sports LLC, walking through or waiting at the premises of Kearney Combat Sports LLC before or after classes have been held. I understand that by signing this release would bar any lawsuit and the courts would dismiss it on the grounds that have voluntarily signed this release, I have assumed all the risks associated with the participation of martial arts activities. This also  includes any kind of negligence on the part of the coaches/instructors of any classes at Kearney Combat Sports LLC, the negligence of any members of Kearney Combat Sports LLC, and premises liability. 

     I understand that if I am signing this release on behalf a minor child that I am giving up the very same rights for the minor child as if I were singing this release on my own behalf. I have read this agreement in full and understand the wording and language in it. I have been made aware of and advised of all the potential risks and dangers incidental to partaking in martial arts training/classes/practices at Kearney Combat Sports  LLC, including the liability of being at the premises. I am in agreement that this release form is a one page document that I have been presented, have fully read and understood the contents which it contains and the rights I am willfully giving up in order to participate at Kearney Combat Sports LLC by signing below as indicated.
